Step 1

We need your proof of vaccination so that we can keep everybody safe and happy. See our required list below.

  1. Rabies

  2. Bordetella

  3. Canine distemper (DHPP or DAPP, or DA2PP)

  4. Influenza

Download and send back the Enrollment Form along with your vaccinations to us at dogpiledogs2018@icloud.com with the subject line New Enrollment.
